DUI Blood Test vs. Breath Test: What's the Difference?
Breath and blood tests are both used in DUI investigations, but they do not collect or measure evidence in the same way. Understanding those differences matters when interpreting the result.
After a DUI arrest, chemical evidence commonly comes from one of two places:
Breath or blood.
Both may be used to evaluate alcohol.
Blood can also be used to test for drugs.
But a DUI breath test and DUI blood test are not interchangeable procedures, and neither is automatically "better" simply because it uses a particular type of specimen.
Each produces different evidence and presents different forensic questions.
What Does a DUI Breath Test Measure?
An evidential breath-test instrument analyzes alcohol in exhaled breath.
The instrument reports a breath alcohol concentration, commonly expressed as grams of alcohol per 210 liters of breath.
Breath testing has several practical advantages.
The sample can generally be obtained relatively quickly.
The result may be available immediately.
Multiple samples can often be collected within a short period.

What Does a DUI Blood Test Measure?
A blood test directly analyzes a biological blood specimen.
For alcohol cases, a forensic laboratory can measure ethanol concentration in the specimen.
For suspected drug-impaired driving, blood can also be tested for substances such as:
- THC and cannabis-related compounds
- Stimulants
- Benzodiazepines
- Opioids
- Sedating medications
- Other drugs depending on the laboratory's testing scope
That makes blood particularly important in drug DUI cases, because a breath test designed for alcohol generally does not determine whether cannabis, prescription medications, stimulants, or most other drugs are present.
Breath Tests Usually Produce Results Faster
One of the largest practical differences is timing.
An evidential breath test may produce a result minutes after the sample is provided.
Blood testing generally involves:

- 03Evidence handling
- 04Transportation or submission to a laboratory
- 05Laboratory analysis
- 06Quality-control review
- 07Reporting
The laboratory report may not be available until considerably later.
But the important forensic timing question is not when the lab issued its report.
It is when the specimen was actually collected relative to driving.
The Delay Before Testing Matters
Neither a breath test nor a blood draw usually occurs at the exact moment the vehicle was being driven.
That creates a timeline issue.
Alcohol concentrations can change during the delay.
Drug concentrations can change as well, sometimes quite substantially.
Therefore, a chemical result must be anchored to:
- Time of driving
- Time of stop or collision
- Time of arrest
- Time of any preliminary test
- Time of evidential breath testing
- Time of blood draw
- Reported consumption history
Without a reliable timeline, it may be difficult to determine what a later measurement establishes about the earlier period of driving.
Breath Testing Has Its Own Reliability Questions
A breath-test review may examine:
- Instrument approval
- Instrument checks
- Quality-assurance records
- Operator qualification
- Observation or deprivation period
- Mouth alcohol concerns
- Sample acceptance
- Duplicate-sample agreement
- Error messages
- Testing sequence
- Breath-test ticket or electronic data
- Video of the testing process
The exact rules vary by testing program and jurisdiction.
The fact that the result was generated by a machine does not eliminate the need to review the process.
Blood Testing Has Different Reliability Questions
Blood evidence raises its own issues.
A review may consider:
- Who collected the blood
- Time of collection
- Collection kit
- Tube identification
- Preservative and anticoagulant
- Packaging
- Chain of custody
- Storage
- Laboratory methodology
- Calibration and quality controls
- Analytical uncertainty
- Whether the reported compound is active or a metabolite
- Whether the laboratory result supports the conclusion being drawn from it
Again, identifying a deviation does not automatically mean the result is invalid.
The question is whether the issue is scientifically meaningful.
Blood Is Not Automatically More Accurate Than Breath
It is common to hear:
"Blood is more accurate."
That is too simplistic.
A properly collected and analyzed blood specimen can provide highly useful quantitative evidence.
A properly administered evidential breath test can also provide highly useful quantitative evidence.
The two methods have different strengths, limitations, procedures, and potential sources of uncertainty.
The correct question is not which technology is universally superior.
It is whether the specific test in the specific case was properly obtained and appropriately interpreted.
Drug DUI Cases Are Different
Blood testing becomes particularly important when drugs rather than alcohol are suspected.
A 0.00 breath alcohol test says little about whether THC, a benzodiazepine, stimulant, opioid, sleep medication, or another substance is present.
But detecting a drug in blood creates another interpretive question:
Does the concentration establish impairment?
For many drugs, that relationship is not as straightforward as people expect.
THC is a particularly important example.
The presence or concentration of THC should not simply be converted into an alcohol-style impairment conclusion.
Drug toxicology must be interpreted alongside the person's driving, physical signs, behavior, testing performance, DRE findings, and timeline.
What Happens When Breath and Blood Results Differ?
Occasionally a case contains more than one chemical measurement.
A PBT may be followed by an evidential breath test.
A breath test may be followed by a blood draw.
When results differ, the answer is not necessarily that one test was wrong.
Time passed between the samples.
Alcohol concentrations change.
Different specimens and analytical methods may use different reporting conventions.
A meaningful comparison requires evaluating the timeline and the characteristics of each test.
Breath or Blood: Different Tools, Different Questions
Breath and blood testing are both important tools in DUI investigations.
But they answer questions in different ways.
Breath testing analyzes alcohol in exhaled breath.
Blood testing analyzes a biological specimen and may identify alcohol as well as drugs.
Neither should be interpreted without considering collection procedure, timing, quality-control information, and the rest of the DUI investigation.
